Useful Patient Information
Preparation
Before your first visit with us, you will be required to complete new patient paperwork which can be filled out just prior to your initial appointment.
Arrive approximately 15 minutes early in order to be ready for your appointment time with your therapist. Wear or bring loose-fitting clothing for your evaluation.
Insurance / Finances
Our helpful receptionist at the front desk will be able to assist you with any insurance or financial questions you may have along the way so that money will never interfere with your recovery.
Our patients are our main focus, and we want you to feel comfortable with costs and available payment options offered by us. We won't let financial worries hinder the progress of your quality physical therapy.

Follow-Up and Communication
Follow-ups require you to simply sign in and have about an hour to spare for each session. These sessions are extremely important and will keep your body recovering productively with a customized schedule and series of appointments. Remember to wear loose-fitting clothing and arrive a few minutes early. We provide a changing room for your convenience.
Because it is an integral part of your therapy, we continue communication with your referring physician to ensure consistent quality care as change and progression occur. You should expect a physician progress note to be sent to your physician each time you return for an appointment with your physician. Your physician or therapist usually determines the number of visits that will be appropriate for your condition.
At Advance Rehabilitation, most insurance policies are accepted.
